Empty Can Test for Shoulder Impingement

The Empty Can Test (aka Jobe's Test) is used to determine a shoulder impingement or a supraspinatus tear. The test can yield both false positives & false negatives, so use it in combination with other diagnostic tools.

The patient can sit or stand for the empty can test. The patient's arm should be elevated to 90 degrees in the scapular plane (about 45 degrees), with the elbow extended, full internal rotation, and pronation of the forearm. Then you push down on the arm. A positive sign is pain and/or the patient can’t hold their arm up.

The test can also be positive for other shoulder issues including shoulder bursitis, tendonitis, or even a labral tear.

Hawkins Kennedy Test for Shoulder Impingement

The Hawkins Kennedy Test is used to determine a possible shoulder or subacromial impingement. This test can yield false positives & false negatives, so use it as one of many diagnostic tests.

The Hawkins Kennedy Test is performed by placing the patient’s shoulder in 90 degrees of flexion with the elbow in 90 degrees of flexion. Then you internally rotate the arm. A positive test is pain with internal rotation.

However, pain can also occur if a patient has a rotator cuff tear, labral tear, or other issues of the shoulder including bursitis and tendonitis.

How to Use a TENS / EMS Unit for Shoulder Pain

A TENS / EMS Unit can be a great tool for helping to reduce and relieve shoulder pain. Here are some pad placements to relieve different types of shoulder pain. 

Some common shoulder injuries that may benefit from a TENS/EMS unit include things like rotator cuff tears, labral tears, sprains, strains, tendonitis, bursitis, SLAP tears, frozen shoulder, shoulder impingement, shoulder replacements (TSA), and general arthritis.

TENS (Transcutaneous Electrical Nerve Stimulation) is used to help reduce pain and increase circulation. Basically the vibration of the TENS follows the same pathway as the pain pathway to the brain and helps cancel it out. It is a great alternative to pain medication. A TENS uses two or four electrodes with cross currents to surround the area. The deep vibration/massage helps relax the muscles and allow for healing. You should not get a muscle contraction when using TENS.

EMS (Electrical Muscle Stimulation) is designed to treat more of the muscle area than the nerves. It helps activate the muscles to help healing, and regain strength in that muscle. EMS is uncomfortable, and you should get a contraction with this. To help with elbow strengthening, placing the electrodes over the wrist extensor muscles is a great option.

Physical Therapy for Neck Pain Relief

Physical therapy for neck pain can help relieve pain from a wide variety of neck issues including bad posture. These neck stretches & exercises can help with general neck pain and neck tightness.

First up we have chin tucks. Chin tucks are my favorite exercise and/or stretch for the neck. It does a great job of “resetting” the neck muscles, and stopping forward head posture.

Isometric exercises can also be very beneficial, especially when you are having a lot of neck pain and big movements increase the pain. Isometric exercises activate or contract the muscles without moving the muscles.

The final two stretches are not only great for neck pain, but they can also help with shoulder and upper back pain as well. The upper trapezius and levator scapulae muscles cover the neck, upper back, and shoulder area, so they can cause a lot of neck pain, shoulder pain, and upper back pain when they are tight.
